Sperm Quality The Essential First Step Toward Solving Male Infertility

What Is Sperm Quality Testing?

Fertility is a shared responsibility between both partners. In approximately 40% of cases, male infertility is the primary or sole contributing factor. However, accurate diagnosis leads to the right solution and that solution begins with the first essential step: evaluating sperm parameters through semen analysis.

Semen analysis is a simple, non-invasive procedure that provides valuable insights into a man’s fertility potential. A standard semen analysis (spermiogram), along with specialized tests such as sperm DNA fragmentation and oxidative stress testing, offers a comprehensive evaluation of male reproductive capacity. Using advanced diagnostic tools, we can thoroughly assess the functionality, viability, and fertilization potential of sperm cells.

At FertUlity, we understand that infertility can be a stressful and emotionally demanding experience for couples. However, our clinic offers a wide range of effective treatments and solutions for male infertility. The first and most important step is early and accurate diagnosis, allowing us to design a personalized treatment strategy tailored to each patient’s needs.

When Is Sperm Analysis Necessary?

Semen testing is an integral part of the basic fertility evaluation for any couple facing conception difficulties. It is especially important:
In cases where pregnancy is delayed for more than 6 to 12 months
When the female partner has a history of infertility or has started related tests
In preparation for assisted reproduction treatments such as IVF
When risk factors exist such as varicocele, inflammation, chemotherapy, or exposure to toxic substances
In cases of previous unsuccessful artificial insemination attempts

What Does Sperm Quality Evaluation Include?

Laboratory testing includes multiple examinations that collectively provide a complete picture of a man’s reproductive capacity and fertility potential:

Semen Analysis

A semen analysis is a standard laboratory test used to evaluate the key characteristics of a man’s sperm. A semen sample is collected after 2 to 5 days of abstinence and analyzed within a specific timeframe to ensure accurate and reliable results.
The main characteristics evaluated include:
Total sperm count, reflecting the testicular production capacity.
Sperm motility, referring to how efficiently and progressively sperm move—essential for reaching the egg.
Sperm morphology, assessing the percentage of sperm with normal structure, directly linked to their ability to penetrate the egg.
Additional criteria include:
White blood cells (leukocytes), which may indicate inflammation or infection in the male reproductive system.
pH levels, which can reveal abnormalities in the seminal environment.
Viscosity and liquefaction time, related to the sperm’s ability to function properly.

Advanced Male Fertility Testing

In cases where the basic semen analysis shows abnormal findings, more specialized tests may be required.
These include:
Sperm DNA fragmentation testing, to assess the integrity of genetic material and detect potential DNA damage affecting fertility.
FISH test (Fluorescence In Situ Hybridization), for detecting chromosomal abnormalities.
Semen culture, to identify infections or inflammation.
In cases of severe oligospermia or azoospermia, genetic testing is recommended, including karyotype analysis, Y chromosome microdeletion testing (AZF), and CFTR gene mutation testing associated with cystic fibrosis.
Hormonal testing (FSH, LH, testosterone), to identify endocrine causes.
A comprehensive diagnostic approach enables couples to receive targeted medical guidance, avoiding unnecessary or inappropriate treatments while maximizing the chances of successful conception.

How Reliable Is the Test?

The reliability of sperm analysis tests depends significantly on the sample collection process, the time of sample delivery, and the expertise of the embryologist or microbiologist. It is recommended that the sample is collected in a designated room within the clinic or transported quickly from home under specific medical instructions to ensure accurate and reliable results.

In many cases, a single semen analysis (spermiogram) is not sufficient to form a complete picture of a man’s reproductive health. A combination of standard semen analysis and advanced fertility tests, along with repeating the test after a certain period of time, helps provide more reliable conclusions. This is because sperm quality can be temporarily affected by factors such as illness, infections, stress, diet, or medication use.

Frequently Asked Questions About Sperm Quality Analysis

What Should I Avoid Before Sample Collection?

It is recommended to abstain from any sexual activity for 2–5 days prior to the test, as well as to avoid intense physical activity, in order to ensure accurate and reliable semen analysis results.

Can stress affect the results?

Yes, high levels of stress can affect ejaculation, potentially reducing the volume of the semen sample and consequently impacting the test results. In such cases, it is recommended to repeat the semen analysis after 3–5 days for more reliable outcomes.

Are There Medications or Supplements That Improve Sperm Quality?

Yes, the use of antioxidant supplements has been positively associated with improvements in sperm quality parameters. Additionally, avoiding smoking, alcohol consumption, and excessive heat exposure may contribute to better results in sperm quality analysis.
